Registration - Two Students
This form is to be completed by the parent, legal guardian, teacher or mentor of the students. Do not fill out or submit this form if you are under 18.

READ THESE INSTRUCTIONS BEFORE PROCEEDING.

If you are not prepared to take a few minutes to complete this registration, do not start the application. Wait until you have the time.

INSTRUCTIONS:
1. Get something to write with and a piece of paper.
2. You will need to be able to print this page as well as others in this process.
3. You will need all information for yourself, your partner if you have one, and your project ready. Look over this page and make sure you have all of the information being asked for before you proceed.
4. Type in information for all fields. Check to make sure all information is correct. Click "submit".
5. Do not submit more than once. If you find you have made an error, you will need to contact the GSSSF Director at gsssf@most.org to make arrangements for corrections.
6. Print out form.
